Protecting the personal information of our customers

This notice also tells you how you can limit disclosure
of your personal information. This notice applies to current
and former customers. We will send our current customers
our most recent notice at least annually. We may amend
this notice from time to time.

that meet or exceed applicable law to protect your personal

Forms. Most of the information we need is included on
the application and other forms you complete.

type of your policy or contract. This may include financial
and medical data, motor vehicle reports, claims reports,
and credit reports. We may also obtain information from
reputable consumer reporting agencies.
Transactions or Experiences. Your file may contain information
based on our transactions and experiences with you, such
as your payment history.
Other sources: Such as your employer and other third parties.
REQUESTING A COPY OF YOUR PERSONAL INFORMATION
To obtain a copy of your personal information, simply
send a written request to the applicable address below.
Please include a copy (not the original) of some type
of personal identification, such as a driver's license.
We will respond within 30 business days after we receive
your request.

We will not send you certain documents related to claims
or lawsuits and medical information received from a health
care provider. You will need to obtain your medical information
directly from the health care provider.
Upon your written request, we will inform you if we obtained
a consumer report about you and the name and address of
the reporting agency.
If you believe any of the information we have about you
is incorrect, please notify us in writing. We will research
and correct any errors we find. If we do not find any
errors, you may send us a statement disputing the information.
We will send the correction or statement to anyone who
received the original information. Please contact us if
you have any questions about your right to access or want
us to correct information in your file.
AFFILIATES TO WHOM WE MAY DISCLOSE PERSONAL INFORMATION
We may disclose your personal information to other Insurance
Designers companies (our "affiliates"). Our
affiliates include life, car and home insurers, securities
firms, broker-dealers, a legal plans company, and financial
advisors.
NON-AFFILIATED THIRD PARTIES TO WHOM WE MAY
DISCLOSE PERSONAL INFORMATION
Non-affiliated third parties are not part of the Insurance
Designers family. We may disclose your personal information
to them with your consent. We may share your information
to service and administer our business as permitted by
law. We require our vendors to keep your personal information
strictly confidential. We may also disclose your information
to reputable companies that market products or services
that may interest you.

in any report
• An insurance support organization or another insurer
to properly underwrite a risk, prevent or prosecute fraud,

prepare a report. As allowed by law, they may disclose

SHARING WITH INSURANCE DESIGNERS AFFILIATES
FOR MARKETING PURPOSES
The law allows us to share with our Insurance Designers
affiliates any information about our transactions or experiences
with you. If you prefer that we not share your name with
our Insurance Designers affiliates to enable them to market
their products or services to you, please follow the instructions
on the Insurance Designers Privacy Statement screen on
this website. If you make such a request, we will not
share any of your other information with our Insurance
Designers affiliates, unless otherwise permitted by law.
